Payments: We require a credit or debit card information to be saved on file while actively enrolled. We will automatically bill the account on file on the 1st of the month. Should you prefer to pay by another method, you may do so before the end of the current month. A late fee of $20 will be assessed at 8:00pm on the 5th of the month if tuition was unable to process on the 1st. Accounts with unpaid balances by 8:00pm on the 8th of the month will be dropped from class. Lifetime Registration Fees: Brand New Families are charged a $65 registration fee that covers all family members and is lifetime and will never be charged again regardless of length of enrollment, number of children, or taking a break and returning. It is a one time LIFETIME fee. Family Discounts: Family and multiple class discounts are automatically applied to your bill. 15% discount is applied to the second class and all classes beyond that are discounted at 20%. Discount applies to lowest cost classes and cannot be used with any other discounts. Private Lessons: Billed by the month and based on the number of days the lesson meets for that particular month.

Drop Policy and Refund Policy: Dropping/Missed Classes: There are no prorated refunds or credits for missed classes or for dropping classes mid month. If you wish to drop the class for the next month, simply tell the office staff prior to the end of the current month.

Medical reasons for dropping a class: Medical situations supported by a written and signed acknowledgement from a licensed medical practitioner will allow us to extend 2 options:
1. Drop (mid-month allowed) and receive a credit for the remaining month paid on your OSA account prorated from the date we received the medical acknowledgment.
2. Remain enrolled to keep your spot, mark your child absent for the weeks they will need to miss and receive make ups on your account for the missed classes. There is no credit to your account in this scenario. Please note that fees are not refunded due to non participation in a class or event.

Consent to Participate: As the adult student or Parent or Legal Guardian of the minor student(s) named above, I consent to their participation in the programs offered by ADAD Trifiro Limited, LLC DBA Ohio Sports Academy, Dragonfly Aerial Arts, or Ninja Zone and to use facilities at Ohio Sports Academy owned by DADA Limited LLC. I understand the nature the nature of the activities and believe myself or my minor child/children to be qualified, in good health, and in proper physical condition to participate in such activities.

Ninja Class Policies: Enrollment in a Ninja class requires the purchase of a Ninja shirt. The shirt must be worn to class as it is considered an integral part of the program and helps to build the class culture. Missions are required to level up to ensure that each Ninja has the necessary skills to move to the next level. At OSA we provide up to two opportunities each month to level up. Your student will receive a postcard from the instructor when it is time for them to attend a Mission.

Aerial Arts: Aerial Arts students cannot participate in class if they arrive more than 15 minutes late because of the time it takes to properly warmup at the beginning of each class. We recommend that Aerial Arts students wear leggings or capris to protect limbs while performing skills. Our Aerial Arts students have the opportunity to perform in a Showcase periodically for family and friends. This Showcase is optional, and a separate fee is charged to perform.

Private Lessons: Private lessons are billed by the month and billing will reflect the total number of lessons in the month. A missed private lesson can only be made up in a class and only if prior notice is given for the absence. You will need to call the office to reschedule the makeup. Refunds are only given if OSA has to cancel the private lesson. We suggest students arrive at least 10 minutes prior to the start of the lesson so they can warm up.

Text Messaging: I agree to accept text messages from Ohio Sports Academy in the event of gym closures, class cancellations, and other gym related issues. I understand that OSA will never attempt to sell anything or remind me of a for pay event via text. I also understand that I am able to opt out of texting and signing this agreement does not override my right to opt out.

Photo Release: I grant to ADAD Trifiro Limited LLC, dba Ohio Sports Academy, the right to take photographs of me and my family in connection with the above-identified event or class. I authorize Ohio Sports Academy, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Ohio Sports Academy may use such photographs of me and my family without names and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

Misconduct Prevention: Coaching sports, particularly gymnastics and sports involving gymnastics-like maneuvers, creates opportunities for physical contact between a coach and a student. Physical contact is acceptable when it is reasonably intended to coach, teach, or demonstrate a skill or to prevent or lessen injury (e.g. spotting, catching). Our coaches exercise extreme care to ensure that such contact is not invasive of sensitive areas of the body. Infrequent, non-intentional physical contact particularly that which arises our of error on the part of the student or coach, does not constitute physical misconduct. I consent to have the coaching staff engage with my child, in any manner that is reasonably intended to coach, teach, or demonstrate a skill or to prevent or lessen injury. Should I have any concerns of misbehavior and/or misconduct I agree to report the incident to the gym's management as soon as possible.

Medical Emergency: The undersigned gives permission to Ohio Sports Academy, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

Acknowledgement of Risk: I understand and acknowledge that by the very nature of such activity, gymnastics, cheerleading, tumbling, Ninja Zone, aerial arts, and trampoline all carry a risk of physical injury. No matter how careful the student, coach or instructor, no matter how many spotters are used, no matter how many mats provided, no matter what type of equipment is provided and no matter how many times the skill has been successfully completed, the risk cannot be eliminated. I UNDERSTAND AND ACCEPT THAT RISK. I knowingly and willingly assume all such risk. Being fully aware of the dangers, I give consent for myself or my minor child/children to participate. I further acknowledge, understand, appreciate and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to MRSA, Influenza, and Coronavirus. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation and exposure.

Missed Class Policy: Recreation Classes: The tuition you pay covers your child's spot in class the month in which it is paid. If the office is notified of an absence in advance, a missed class may be made up in a class if there is space available. Private Lessons: Missed private lessons may be made up in a class if we are notified in advance of the absence. You must be enrolled in the gym in order to use any makeup classes.

Release of Liability: In consideration for my(if over 18) or my child(ren's) participation, I (we) hereby for myself and my child(ren) and our respective heirs, executors and administrators, COVENANT NOT TO SUE and FOREVER RELEASE ADAD Trifiro Limited, LLC (Ohio Sports Academy) DADA Limited, LLC (the landlord) and the Ninja Zone, and or Dragonfly Aerial Company, the owners, operators, directors, officers, employees and other members of the "releases" named above, from personal injury or accident of any sort or nature suffered by me (us), the undersigned, by reason of participation or membership in classes, lessons or any programs or activities of Ohio Sports Academy(ADAD Trifiro Limited LLC) Dragon Fly Aerial Arts or the Ninja Zone including those resulting from acts of negligence. I also assume all medical expenses for the aforementioned child or myself, which may be the result of any injuries sustained while training at, or performing for Ohio Sports Academy (ADAD Trifiro Limited LLC). Further, it is affirmed that sufficient insurance covering all such injuries and damages shall be in full force and effect throughout the program or its equivalent throughout the child's/parent's gymnastics career by my insurance.
